1st November to 30th November 2025 - Community Out Reach Activity

This report outlines the comprehensive community health outreach initiative undertaken by the Masters in Community Health Nursing students. The primary objective was to deliver "doorstep health care" to rural populations in Bonglikech and Riwarla. The outreach focused on a lifecycle approach—addressing health needs from newborns to the elderly—bridging the gap between the community and essential primary health services.

2. Objectives

● To assess the baseline health status of the community through comprehensive screenings.

● To provide age-specific care interventions (Maternal, Child, Adolescent, and Geriatric).

● To detect communicable and non-communicable diseases early to prevent complications.

● To promote health awareness through School Health Programs and nutritional counseling.

3. Comprehensive Health Services Delivered

A. General Health Screening & Diagnostics

A robust screening station was set up to evaluate general health parameters for all age groups.

● Vitals Monitoring: Blood Pressure (BP) monitoring to identify hypertension cases.

● Anthropometric Measurements: Body Mass Index (BMI) calculations to assess malnutrition or obesity risks.

● Lab Investigations: Hemoglobin (Hb) estimation to detect anemia.

● Disease Screening: Targeted screening for common Communicable Diseases and Non-Communicable Diseases (NCDs), alongside preliminary risk assessments for cancer.

● Nutritional Assessment: Dietary recall and counseling for families identified with nutritional deficiencies.

B. Maternal & Child Health (MCH)

● Maternal Health Care: Antenatal and postnatal assessments were conducted for eligible women, focusing on high-risk pregnancy identification and iron/folic acid supplementation advice.

● Newborn & Under-five Care: Growth monitoring (weight/height), immunization status checks, and assessment for common childhood illnesses (ARI, Diarrhea).

C. Vulnerable Group Interventions

● Adolescent Health: Counseling sessions on reproductive health, menstrual hygiene, and mental well-being.

● Elderly Care: Geriatric assessments focusing on mobility, sensory impairments (vision/hearing), and management of chronic pain and NCDs.

D. Institutional & Home-Based Care

● Household Visits: Students conducted door-to-door visits to ensure coverage of bedridden patients and those unable to attend the central camp.

● School Health Program: A dedicated session was held at the local school involving physical check-ups, hygiene education (handwashing techniques), and de-worming awareness.

Conclusion

The outreach program successfully met its goals by providing accessible, high-quality care to 200 individuals across Bonglikech and Riwarla. The initiative not only facilitated early diagnosis of health issues but also empowered the community with the knowledge to maintain their health. The hands-on experience provided invaluable exposure for the Masters in Community Health Nursing students in managing rural health dynamics.
